  1. Welcome to your online MC report form. This is probably the most critical element of our Quality Control programme.

    This report must be submitted as quickly after each formal as possible. If phones are going to ring at 9am the next morning, we need to have these answers in advance, good or bad.

    Please fill in the following form thoroughly.

    If any element is not applicable, you will have the option to state that.

    If each element went to specification, a simple tick "yes" will be enough but if it's "no" you should make comment.

    If anything is missed or incorrect, the form won't submit the data until every question is answered properly. Missing or incorrect data will be marked in red if the form doesn't send.

  16. TEAM MEMBER DUTY PERFORMANCES - The quality of performance of each team member in their own role is critically important.

    Team members here also include venue staff such as waiters, Chefs, supervisors and others.

    The quality of their work is what we seek to get information on. For instance, if the food is not up to standard, this would be a failure of the performance of Chef. If nobody was dancing, this would be a failure of the performance of the DJ.
